Reform of the Healthcare System as a Basis for Streamlining the Organization of Circulation and Availability of Medicines for Patients in the Context of the COVID-19 Pandemic

Abstract

The complexity and unresolved many problems in the organization of circulation and availability of medicines for patients in the context of the COVID-19 pandemic necessitate the development of rule-making practice and substantiation of scientific principles in this area on the basis of pharmaceutical law. The aim of the work was to improve the normative practice of organizing the circulation and availability of medicines on free and discounted prescriptions for outpatient treatment of certain groups and certain categories of diseases during the reform of the health care system in the COVID-19 pandemic based on analysis, generalization, systematization, as well as the development of rule-making measures using an integrated approach. More than one hundred normative and legal acts (laws, resolutions, orders, orders, instructions, instructions) on the chosen topic have been worked out. The study of the principles of scientific substantiation of normative measures for the organization of circulation and availability of medicines for patients was conducted in the areas of organizational and legal, regulatory, evidentiary, clinical and pharmacological, formulary, forensic and pharmaceutical, scientific and methodological, social and personalized directions. Used methods of documentary, normative and legal, comparative, system, generalization and graphic analysis used. It has been proven that it is impossible to carry out qualitative reforms of the healthcare system at a rapid pace (4-5 years). Ukraine has begun reforming its health care system. A number of measures have been taken to reduce the state budget spending on health care without the necessary preparatory work: study the effectiveness of the existing system, study domestic and international experience, the necessary calculations with the involvement of specialists in various fields (scientists, doctors, economists, etc.). The achievements of the domestic system are pointed out and attention is also paid to its shortcomings. The population of the country is not sufficiently informed about the changes in the health care system and the possibility of receiving free medical care and medicines. It is noted that the path from a family doctor to a specialist significantly complicates the possibility of receiving emergency free medical care and leads to corruption. According to the results of the research, normative initiatives and editorial changes to the resolutions of the Cabinet of Ministers of Ukraine were proposed. The prospects of further research on the organization of circulation and availability of drugs for different contingents of patients in a pandemic COVID-19 are proved.
